We require a credit card on file to book an appointment online. If we don’t have that information already you will be asked to enter it below* after selecting an appointment time. You will not be charged at the time of booking, however, if we don’t receive at least 24 hours (48 hours for Monday appointments) notice of cancellation or need to reschedule, the card on file will be charged $100 for a NEW patient missed appointment and $65 for a RETURN patient missed appointment.
*Credit card information is encrypted and stored in our secure payment system. Staff/employees can not see credit card numbers.
